San Francisco police shot and killed a man Wednesday they say was firing a shotgun at a construction site outside a Mission District hospital.

The San Francisco Medical Examiner's Office identified the man as Javier Lopez Garcia, 25. The office originally said he was from San Francisco, but later identified him as a San Jose resident.

According to the department's account -- see the complete statement below -- at about 4:15 p.m. Wednesday, officers responded to a report of a man carrying two firearms at a building under construction outside St. Luke's Hospital at Valencia and Cesar Chavez streets. Officers spotted Lopez Garcia with a long gun atop a construction elevator adjacent to the medical facility, the department said.

Police said three officers opened fire when the suspect, dressed in white coveralls, pointed his weapon at them. He was fatally wounded. CBS5 video of the aftermath showed an officers tossing a flash-bang grenade onto the elevator in an apparent attempt to make sure the suspect had been disabled.

Police said Lopez Garcia is believed to be the same man who had stolen a shotgun and ammunition from a San Bruno sporting goods store shortly before the Mission District confrontation.

Speaking to reporters near the scene of the shooting, Police Chief Greg Suhr said Lopez Garcia's motives were unknown.

"We have no idea why he did what he did at the construction site," Suhr said.

The episode marked the fifth fatal shooting by San Francisco police in 2015.
